The steps to ask a doctor´s appointment in the centres associated to Santiago Health are the following:

Step 1: Fill out the Form of Doctor´s Appointment
To evaluate your situation and chose the more suitable specialist, you will be asked to fill out the Form of Doctor´s Appointment. Here you can give us information about your symptoms and health condition, information to contact you and your date of preference for the doctor´s appointment. This form will be received by a Coordinator of the International Office, who will guide you in the process and will be from now on your main contact with the health provider that you have chosen. It is important that you complete all the requested information to speed the process up.
Make sure to give precise and clear information about how to contact you by email, telephone or fax.

Step 2: Send medical reports, images and exams
Alter sending the Form of Doctor´s Appointment by fax or by email, you will be asked to send the Report to your Treating Physician in Spanish together with images and complementary exams when necessary, by email or courier. You should send this information to your Coordinator of the International Office, to the addresses indicated in Contact Details International Office.

Step 3: Confirm the Availability of the Appointment
Alter the Form of Doctor´s Appointment and medical reports have been received, the Coordinator of the International Office will contact you to inform the alternative dates, available time for your appointment in the center chosen. Besides you will receive a preliminary estimation of the budget for the medical assistance which should be covered in advance by means of some alternatives indicated further in order to get effectively the reservation.
